ENTRY FORM

INSTRUCTIONS FOR COMPLETING THE FORM:

Please pay close attention to filling the FAMU ENTRY FORM and provide all the information required in Czech and English. Please, in addition to the synopsis, fill in the logline and the director's note - for many festivals it is a mandatory information in the application and the festival coordinator will not put this together for you. It is necessary that she has everything available and then she no longer has to contact you when filling out individual applications - it is a big and mutual time saving.

- Send the completed application form together with other materials needed to register the film for the festival to Alexandra Hroncová by e-mail to alexandra.hroncova@famu.cz
- The form cannot be filled in in a browser (at least on the MacOSx platform), ie. it is necessary not to open it from the link, but to download it as a file (MacOSx - Ctrl + click => Download Linked File) You can fill in the form via applications, see next point
- The form can be filled in using Adobe Reader DC (Windows / MAC), or in the same application for the Android or IOS (MAC) platform in Google Play or Appstore.
- Filling in all fields of the form is mandatory.
- After filling out the form, save it and send it to email alexandra.hroncova@famu.cz. If you use a mail client like Outlook, etc., you can send it directly through the dialog at the end of the form.
